This combined document ensures your healthcare preferences are honored and provides peace of mind for you and your loved ones. Without a Healthcare Power of Attorney/Living Will, decisions concerning your medical care (if you are in a coma or permanent vegetative state) will be left to your spouse or family members. Your wishes may not be followed without written guidance.
WHAT YOU’LL NEED TO COMPLETE OUR DIY FORM:
To complete our DIY form, you will need to provide the full legal names, addresses and phone numbers of both yourself, your chosen agent and a successor (back-up) agent. If you don’t have a back-up agent, that’s ok. You’ll also need to clearly articulate your preferences for medical treatment in various scenarios. MINIMUM REQUIREMENTS:
You must be at least 18 years old and be of sound mind.
You must sign and date the document.
Two individuals must witness your signature and they must be at least 18 years old.
The Healthcare Power of Attorney/Living Will does not need to be, but should be, notarized.
